The official OHSBL or LCHS Form must be used. These are different versions, one for baseball (OHSBL) and the other for softball (LCHS.) To obtain a copy each season, email Pat Anderson (pat@ohsbl.com) and the form will be shared with you. This is a Google Sheet, where you will be required to enter:
New for this season, all releases must be completed electronically. The link below is to be used only by community personnel directors. Each player needing a residence (community) or age release must complete one form per player.
This is a list of those with valid background checks, along with the date cleared and File Number. This is for reference only. If someone is not on this list, they are either expired or have no background check. If someone has a "Flagged" or "Cancelled" decision, they are NOT permitted to coach.
This form must be used when turning in a community's rosters. Use additional sheets as needed but please ensure all information is filled out completely and accurately. Each version will automatically populate the fees for each level as well as automatically total all fees.
PLEASE MAKE 2 COPIES OF EACH. ONE COPY GOES TO THE TREASURER (WITH CHECK) AND THE OTHER TO THE SECRETARY!

In order to accurately place teams into competitive regular season divisions, please provide details about each team. Please complete one sheet per team and attach as a cover page to each roster.
